HVAC Service Agreement Sign Up Form

By submitting this form, the customer authorizes EMCO Tech, also known as EMCO Tech Heating & Cooling and Emergency Maintenance Company, LLC, to process recurring payments for the selected maintenance plan using the payment method provided. The selected maintenance plan includes an initial 12-month commitment. After the initial 12-month term, the plan will continue on a month-to-month basis unless canceled in writing at least 15 days before the next scheduled billing date. Recurring payments will be processed according to the selected plan. If a scheduled billing date falls on a weekend or holiday, payment may be processed on the next business day. The customer confirms that they are the authorized cardholder or authorized user of the payment method provided and agrees not to dispute charges that are processed according to the selected maintenance plan and the terms stated here. Receipts may be sent by email, and charges may appear on the card statement as Emergency Maintenance Company, LLC, EMCO Tech, or EMCO Tech Heating & Cooling. EMCO Tech may notify the customer in advance of changes to the billing amount, billing schedule, service terms, or maintenance plan details.

EMCO Tech may contact the customer by phone, text message, email, or voicemail for service-related and account-related communication. This may include scheduling and confirming service appointments, sending service reminders and updates, providing invoices and receipts, discussing billing matters, following up on service requests, and other client communication related to the selected maintenance plan or other EMCO Tech services. EMCO Tech may also occasionally send seasonal reminders, service notices, specials, or limited promotional offers related to its services. Message and data rates may apply for text messages. The customer may opt out of promotional text messages by replying STOP and may request to stop receiving promotional emails by following unsubscribe instructions when available or by contacting EMCO Tech directly. Submitting this form does not guarantee acceptance into a maintenance plan until EMCO Tech reviews and confirms the information provided. EMCO Tech may contact the customer if additional information is needed before the maintenance plan is approved or activated.
